Yes it does come with cover and pump.

It comes with a pump, e ordered covers. I don't think it came with one. Be sure to keep adjusting the air until comfortable. It will be a real blessing for the user.

I'm not really sure, the person that is using it weights around 250 pounds.

Q: can you rotate this so it becomes longer rather than wider? we need 16 x 18, not 18 x 16.

It's designed (along with directional arrows on pad) to fit a certain way, but I see no reason it couldn't fit otherwise. Just make sure you can get to the air fitting easily.
