It's on the disk but you need download it from the disk when loading the first time. You don't need any codes.

Considering how long this game has been out, how buggy is it? what are some steps to take to make it less likely to bug out?

For the first maybe 20 hours of game play this game is virtually flawless. Loading screens are short no lag, however after that the game becomes increasingly more problematic, until a point when the game is nearly unplayable. For me I had come to the point when the game would completely freeze my ps3 only mins after starting game play. This really started after mostly completing the main game and started on the DLCs, I got to the second DLC and had enough of the freezing. Ive done a bit of research, and this problem is due to the engines that make up the game, basically its the software programming in the game. From what ive found there is no cure for this Game, however Bethesda has fixed this problem by completly recreating there engines and is seen in Skyrim (which i have and it runs great!). I dont really have any feed back on how to help the bugging out just when your playing and it freezes just let it sit for a bit, it might just need to load. If nothing happens in a minute or two then you will have to restart your system.

No. One player only, and this game can run anywhere from 80-400 hours of playing time. It depends on how much exploring you do. (Although the more you play, the bigger your game file size will get, and the bigger it gets the more you run into glitches. But despite that, this is a great game.) If you don't have much time to get your gaming on, you might want to pass. If you do have the time, this will be one of your favorite games of all time. (I'd recommend playing Fallout 3 GOTY edition first though.)
