Worked for me.
When I got up in the morning, I could not bend over and had burning pain down my right leg. Was almost impossible to even dress. I saw a video on YouTube (Bob & Brad) and they were showing possible things to do to help this kind of back pain. I did the stretching and it did help. I ended up buying this lumbar pillow. It was a big awkward to start, but after about a week, I had no more pain in the morning. I could actually enjoy my shower. Worked for me.

Bad design, very uncomfortable
I’ve bought this item 4 times over the past 4 years and each worked well, provided good support and helped me sleep much better without back pain. But the current version is awful. For some reason, somebody changed the design and this roll is now too big in diameter, too firm and too small in length (though I bought the exact same size as before)—pretty much unusable. Very disappointed.

Woke up with more pain in spine and legs after using this
First, I didn't realize I could not return this when I ordered it. That is on me, lesson learned. This pillow may work for some, but it has caused me more pain in my spine and legs that even Gabapentin won't help. I felt like I was sleeping on a hard roll, not something that made you feel comfortable while giving you support if you are a side/back sleeper. Kept waking up due to the pain and being uncomfortable. So be sure this is what you need before ordering it as $40.00 is a lot to throw away if it doesn't work. The pillow itself is of good quality and worth the money, just didn't help me with my back and nerve problems. I wish you well in finding relief from your back pain. If the pillow works for you it is a 5 star, if it doesn't it is a 1. Being fair I gave it a 3 because it was a zero for me, but others might find relief and give it a 5.

Keeps the curve in your lower back while you sleep
I bought this after my PT recommended it. I don't tie it around my waist, just lay it flat on the bed, parallel to the headboard. Then get in bed, sit up and reach behind me to slip it under my lower back arch before laying down on it. It is very soft so it compresses to fit perfectly in the small of my back. Relieves pain as I sleep. Much more comfortable than the typical half moon bolsters that are too hard and too big. Only con: it would take way more patience than I have to get the tube pillow case on it, because it's too tight a fit. Haven't missed it though. Highly recommend this pillow.

In theory, love the idea, but this particular product did not work for me
pad too firm for me, and too large of diameter to fit the small of my back well. I am 5 ft. 5 and weigh about 130. It just felt too big, and too firm to be comfortable. I ended up cutting part of it down, to see if making the diameter smaller helps... it still seemed a bit firm. I have had more success with rolling a small "chucks" pad into a roll and laying on that. I guess the small of my back is just very "small." :-) I DO note, that sleeping with something in that lumbar curve definitely DOES help me feel less achy, stiff, and painful in my low back upon waking in the morning. Having had previous L5-S1 micro-discectomy, spinal health is very important to me. In theory I love the idea of this strap, but it did not work well for me. Maybe it would work better for others based upon their particular anatomical dimensions.

Seems to have fixed my back
Maybe it was coincidence but from the first night I slept with this thing under my waist, my back pain was 95% better. I have degeneration at the L5-S1 level and suffer bouts of significant pain. I usually have to treat it with drugs and chiropractic but this latest round, I bought this roll and the very next day the pain was almost all gone. I'm going to keep using it. I subtracted a star because the cover isn't quite long enough but that's a minor thing. I got the largest size, I don't know if the cover works better on the shorter versions. I don't tie it around my waist but just lie it on the bed and sleep on it. If I'd known that I would have gotten a shorter one. I assume the foam diameter is the same for all the sizes. I seem able to roll over in my sleep and get it repositioned just fine. It is firm enough that I don't think it will flatten too much very soon. It's not the most comfy thing in the world but I have no trouble falling asleep on it, I get used to it and forget about it in a few minutes, and no back pain is well worth it.

Overpriced bit of foam that has no give and hurts my back
This product is ridiculous. It costs a ton, but it's really only a bit of foam with an ill-fitting cover. It's far too large and too firm to actually fit under the lumbar spine when one is sleeping. It left my back hurting more than before. Additionally, you can't even return the item. Be aware before you buy that there will be no trying it out and then returning it for a refund. You will be stuck with your overpriced strip of useless foam. Oh, and it ties onto your body in a very unhelpful way, because it's hard to tie it tight enough to stay put. That equals lots of wake-ups during the night when you will be forced to wrestle with this bulky girdle that prevents you from easily turning or changing positions in bed. Don't waste your money. You'll find yourself awake and uncomfortable all night, with a painful back in the morning.

It’s helping
I’m putting this darn thing on every night. I’ve been using it about a month now and thought I’d only use it temporarily to get me through a rough pain patch - but I’m continuing. It is actually helping, even though it’s awkward to roll around with. Its main purpose is to support your mid-body while side-sleeping - which is mainly how I sleep. I’ve had back problems for decades and do 1 hour of back exercises a day to keep things under control. Glad I bought it.
