Q: Can it be used as extruder motor for the Creality Ender 2?

I believe it is the same motor.

If you want to use in Creality S4 S5's Y shaft which power is used for Heat bed, very sorry the Motor 42 can not enough power to support, we suggest you can search for Motor 60. If you want to replace in Hotend, and it is no problem. Hope it can help you. Need more further please feel free to email us.

Hello, the motor can work on 5v .7A power supply.

Q: The cable has no guide for which way to plug into my ender 3 mainboard. any tips? if i plug it in backwards do i risk damage?

The non-twisted end goes into the motherboard. It won't damage anything if you do it the other way but try not to do that. Also, the twisted end that goes into the extruder motor needs to have wires repositioned to correctly work.
