Has anyone put these on top of a Bilstein 5100?

A Bilstein 5100 utilizes the same Top Hat on the Coil-over assembly as the factory shock. This will not affect the fitment of a spacer. It will work fine.

Will it fit a 4wd 2019 Ram Laramie Longhorn?

We just put them on my 4wd 2019 Big Horn so I would say yes as long as you don’t have the air suspension.

Do these require an alignment after installing?

Geometrically speaking you cannot change one angle without affecting another. The answer to the question is by raising the ride height you are changing the angles between the ball joints ( caster & camber ) when to these change the toe in changes. The true answer to your question is you should have an alignment performed.
