A: There are 8 simples steps: 
1. Align paper with corner guider and punch.
2. Rotate paper to next corner and punch.
3-4. Punch the remaining corners the same way.
5-8. Flip paper and punch 4 times as in step 1-4.

1.  Clear out punch frequently to avoid clogging.
2.  Remove excess corner squares between 5-8.
