No adhesives, just the sock holding it on. Every now and then I forget to put it on under my sock and about 10 or 11am I think, dang, my leg/ankle hurts again. But I put the patch back under the sock and the rest of the day is comfortable. BTW, I've had leg and ankle xrays, CT scans, ultra sounds 2x.
