How easy is this to take in and out of the macbook pro? my friend has a similar one and he has to use a knife to pry it out. i cringe at the thought

I have found it quite easy to remove the BaseQi from my MacBook Pro SD slot. I have only ever used my finger nail. It fits securely in the slot and because of the metallic end cap, it blends seamlessly with the surface of my MacBook Pro making it esthetic as well as functional! I am very pleased with mine.

Will it work with the sandisk ultra 200gb micro sd

Works like a charm with it, I've got a bunch of microSD cards around to use with GoPro's. Every single one works effortlessly, including the Sandisk ultra 200GB card.

Will this (or a different sized model) fit the Microsoft Surface Book?

No, the 303A model will NOT fit the Microsoft Surface Book (I bought and tried this myself). You need the 103A model (which is a bit longer and fits the Surface Book).
