What is the thread size on this?

I do not know the thread size; it looks to be a 3/4" opening for the nozzle. If you have a hand held shower that you attach to a hose currently, then the thread size for hand shower is identical to the douche nozzle. I use a hand shower and the douche nozzle interchangeably with a shower hose, so fittings and sizes for many of these type items is standardized.

Is it a constant flow of water or is there a button to push to spray?

The system I have has a know which control the constant flow of water. If not use properly you can hurt yourself.
