If you put this on your skin or lips, how do you dillute it, and how do you use it exactly?

Dilute with glycerin, start with 50/50 and work your way up to a 80/20 DMSO/Glycerin. This will prevent any irritation to skin. Apply to skin, wait 5 minutes before rinsing or wiping off. Use up to 5 times daily as long as necessary.

What is the other 0.1% of its contents (non-dmso) composed of?

DMSO can bind to water in the air, so it's impossible to have 100% purity DMSO once it is exposed to air. The longer the bottle is open the more it absorbs water from the air slowly reducing the "purity" over time so keep your bottle sealed!

Is this to be used with a carrier of some sort? water? aloe vera gel? how is it to be applied?

DMSO is an excellent transport medium, nothing else required. Anything mixed with it will enter the body. You want clean skin and only and things you would not hesitate to ingest. Can be diluted with either pure water or aloe vera gel (food grade).
