The package does have only the wheels. There are different axle kits available. Please review model number D 1861.

Is the bearing 100% flush with the wheel or does it stick out even a little?

Yes, they stick out enough that if you bolt it to something the wheel will spin. I would say somewhere in the .030" to maybe .0625" of space between the center hub and the wheel.
