How do you know the size for heating?

Not sure what you meant by heating, it's a cold pack. But if you mean size in general, it has a velcro type back strap and you can adjust it. I found it very handy and always keep in the freezer.

Will this fit a small 20 inch head?

Yes, it definitely will. I have a pretty small head and it fits me perfectly.

Can I order extra gel packs for this producr?

I'm not sure, I just ordered a couple extra of them to have on hand.

Could you use this around your face for wisdom teeth surgery?

Yes if used for one side of the face. The iced part is not that long. And it melts quickly. I wouldn't recommend it for wisdom tooth pain.
