Would this work if I mounted it on a window that is on my roof at 45 degrees?

I would expect that rainwater would pour in through the vent if mounted in that position.

Does the fan consist of two pieces of plastic of larger than 162mm diameter screwed together that clamp across the window aperture?

I have a Ventamatic fan in my shower room. It does indeed consist of two pieces of plastic screwed across the aperture. It is about 180 mm inside and slightly larger outside. I am very pleased with it. I have a storm cover for it but do not use it as it is never stormy here. I recommend this product. Angie
