Can the phone holder rotate from landscape to portrait?

Yes the phone holder can rotate from landscape to potrait easily.

Its pretty much assembled all you have to do is screw the ring light to the tripod and plug it in. Also it has different settings for color and brightness. For the price its held up to my abuse and it also comes with a nice storage bag for the light

The description says that the light rotates 360° but does the light also tilt front and back?

Mine does and it the tripod pivots to whatever position you need it to be.
