I have bsnl broadband connection with dsl modem, now i want to optical fiber via cable to dsl modem, can this conector helpful?

no this not your interet provided cable it poe power over ethernet used for ip phone ,ip cameras

Will this work if i supply an input of 12v or 24v dc?

Probably it will not work. The power adaptor output for this device is 48V 0.5A. The higher voltage is needed because of loss of voltage occurs while traversing through the Ethernet cable.

does it come with power adapter as well? 48v? or we have to procure that separately?

Yes the power adapter is included. You need not buy anything in extra.

Can i use this for ip camera 12v poe input? If we give 12v from input side it will deliver power 12v to otherwise?

Yes. As much as I understand your question you can use this to run ip camera. You can check on its adaptor for the input voltage
