Does this support burst mode on iPhone?

no.. it is not as useful as i thought it might be.

Does this support iPhone 14 pro max? It’s a pretty heavy phone.

It supports my S22 ultra plus case, and I believe it's a heavier phone, so it should support your iPhone 14 max no problem

Will this work with a case with add-on lenses?

Certainly works with a case and add on lenses should not have an affect.
