I am a D so they do fit in all my bras. I will say a trick that might help is to buy 2 sets of the smaller size and stack them so that you don't end up with them peeking out if the demi doesn't cover them comfortably. We used to do this back stage at fashion shows and it works very well. Just stack them like an oreo and put them stacked in the outer side so it lifts and pushes them together. Make sure you adjust your straps a bit (if you don't have a pocket for them in the bra and your straps are too tight they can pop out the bottom but of you adjust they will stay put all night). For breasts 40 and larger youd need even bigger inserts......for most women that large, you should consider the KIND of bra needed.
