what is the difference between Pedag 142 vs. Pedag 127/128 vs 17947 and 17992 ?

It looks like they start to add model numbers to their product, so that's why some same products sound different. Out of curiosity, i bought all three products - 142, 128, 17947. Personally i do not recommend 142 (too thin, not much support). I like 17947/92 best. Thin but has more support. 127/128 has hard heel about a 1/4" thickness tapering down toward the front.

Will these fit inside my athletic shoes or will I have to take out the existing sole?

they are pretty thin I would say you can use them with your existing soles. I use them in my flip flops. I have found them useful and will most likely buy more

Where should the padded hump be placed? directly under the ball of the foot? just below the toes? the arch? i'm so confused. not comfortable today!

It should be right below the ball towards the top of the arch. I was told this by someone who made custom orthotics. It is not suppose to be under the ball like you think. the placement should gently lift the ball to lesson the pressure on it. I have a mortons neuroma and it is uncomfortable but I find these a bit helpful. I have tried many . I feel for you !!!!!
