Q: How to detach the attached cap? Seems loke I have to cut the rubber to take it apart.

Its silicone. Pull on it to stretch it a little bit wider to pull over the bottle opening. Pretty easy. It's the same technique to replace the tops on Nalgene bottles.

Q: Fits in Nissan Rouge 2015 cup holder?

If your cup holder is 3 inch across the bottom, it should fit. I have a a buick and mazda and it works in both. HOWEVER, this seller is providing junk that is not an insulated bottle. Get the identical looking product from Sendestar. I tried this seller twice and got garbage both times.
