Q: does it come with one or two

They come in pairs, I ordered 4 and now have an extra set of 4.

Q: How many chains come in a set? 2 or 4

Each set is for 2 wheels. I bought 2 sets so that I can chain up all 4 wheels in 4WD. I have LT265/75R16’s and they fit perfect. The cam chains are nice because I can get the chains very snug and tightly fitted. I also bought 2 sets of the blue 6 point bungee’s. But probably don’t need them since the cams take all of the slack and slop out of the chains.

Q: will they work on 18 inch tires

I have them for 20" rims. Order by the size of your tires.

Q: Do they work on 20" tires?

Yes, as long as you get the proper tire size chains. I have 20" rims. You'll also want to use tensioners to take out the slack.
