Q: Will this work for shoulder pain too?

I don't think so. It is just for neck and nerve pain in the neck and lower scalp area.

Q: Which direction do you align with neck? Do you align the curved side?

The curved side goes along the base of your skull.

Q: Is the effect of this any different from a still point inducer or one of those over-door, hanging-bag traction devices?

I don't know what a sill point inducer is, but it is not like traction devices. It is specially designed for Occipital nerve issues. I didnt find it to relieve anything else.

Q: Will it work in bed or in a chair?

I use it in my bed every night and it works very well. My chair... not so much, but I do have an adjustable bed so I put it in a recline position and it works exactly like I am on the floor or other flat surface.
