Do you have to wear a wig cap with this?

I don’t like these. Too tight and not adjustable. Don’t like the feel of the material. Think you’ll be much happier with the Dream lover brand. Do not need a cap with either unless you have long hair you need to coral.

Will this fit a 5 year old?

I do not think I would allow a 5-year-old to wear it as it can be tight. You can ask the vendor for the exact dimensions. I measured my band and it is a tad bit more than 20inches. When I wear it sometimes it does hurt my scalp - and not think the pressure would be good for a child. There are softer velvet grip bands which I would suggest.

How can yoy tell both sides look the same?

One side is more pronounced/grips are longer and higher vs. the other side appearing almost flat. You can feel the difference by using your fingertips. If necessary, look through a magnifying glass or blow it up using your phone camera. Hope this helps.

How do I tell which is the inside and which the outside?

The grips are on both sides to hold the wig in place and for it not to slip off your head. There’s no difference in my opinion.
