Q: Can the fans be powered by simply using the USB plug and port or does the big plug also need to be plugged into an outlet?

You need to plug the power supply into an AC outlet. There are two reasons for this: 1) The fans, connected directly to the TV, may draw too much current for the TV's USB circuitry to handle. My USB controller only uses about 30 milliamps (a small amount), when plugged into the TV's USB port, whereas the fans can use up to 500 milliamps. 2) USB voltage is 5.5 volts, so you would be limited to fairly low fan speed. With the multi-voltage power supply, you can select various speeds, like 4.5, 6, 7.5, 9 or 12 volts.

Q: Is there a reason that the fans pull the hot air out, instead of blowing cool air in?

It is generally more effective to pull the hot air out, near the top (where hot air gathers, like a hot air balloon). Pulling the air out should cause cool air to be drawn in from vents lower down. That being said, there might be a situation where you would want to force cool air inside, or have one intake and one exhaust. In that case, it is very easy to change how the air flows on the fan units, by undoing the screws holding the base and top grills from the fan, then turning the fan over and reassembling.

No, the fans will turn off as soon as the USB port signals that it is off. However, the TV components should already be cooler as a result of the fans running before shut off, so that may not be a problem. As an alternative, I also have a TV cooling system, with an adjustable thermostat: https://www.amazon.com/dp/B011M698A8 You would attach the thermostat over a warm spot on the TV (usually over a small vent area), then set the on/off temp. The fans only turn off after that spot has cooled down.
