Title and photo say 10 mm thick. description says 2 inches. which is it??? 10 mm does not equal 2 inches!

It's not 2 inches. A half inch is more like it ... so 10mm is far more accurate.

Could you put it over a a soft mattress to make it firmer?

You could but I don’t think I would personally as it has a chemical rubber type smell and whilst it’s fine for exercise I wouldnt want to sleep with it

What is the measurement from the center of one grommet hole to the center of the other?

Approximately 24.5" from center of one to center of the other Grommet holes are about 5/8" each

Does it work on carpet?

I use it on a hard floor, but I see no reason why it wouldn't work on carpet.
