Will this fit the 2004 rx8?

Yes. I purchased one for my 2005 RX8. One caveat however; I believe the entire assembly is a drop in replacement, but I only installed the lock actuator itself (the black box with the electrical connector) as the rest of my door latch was still good.

Will it fit 2012 Mazda 3?

It won't fit 2012 Mazda 3, the item will fit Mazda 6 2003 - 2008.

I had a bodyshop to install mine.
