About how long do the ice packs last before they need changed?

In a hot room with a window open and about 90 degrees inside these ice packs lasted 3 hours while in the vest. If you use the ice packs without the vest they last about two and a half hours. The cooling time will vary depending on the temperature around the packs and if you use them outside or inside. I would suspect that if you were to use the vest outside with all the ice packs on a hot day these would cool for about one and a half hours maybe less. For $30 the product does the job well.

What sizes are there?

It comes in one size with adjustable velcro straps. I wear a 3x womens shirt so I would look on a size comparison chart before purchasing. You can always add more velcro. I live in a desert state and it works very well for me. I hope this helps you when choosing what you want to do. Have a good summer.

Is there a way to get the water back out of the packs? I want to travel with them after using them.

The water is in the bags permanently. For travel I use ziplock bags and then fill them with ice.
