Q: Is there a way to use top right button & bottom left together (& vise-versa) to make a new command for the Kensington Expert Mouse?

You must install the Trackball software, which starts every time you reboot. it's a free download from Kensington's web site. With that you can define a function when you click both buttons together, and also define what each button does on its own.

Q: Does it have Windows 8 (specifically 8.1) support?

It works fine with my 8.1 x64 install

Q: Are these easier on the wrists considering that the pictures show hands in the horizontal position, which is usually bad for your wrists.

I've been using a variety of trackballs (finger operated not thumb-balls) for months now and I can't go back to using a mouse. i've had literally zero wristproblems since switching for everything (even gaming) and whenever I'm forced to use a mouse even for a short time now I am quickly reminded of my arm / wrist aches again.
